Blumensaat’s Line

Blumensaat's line is the radiographic line representing the intercondylar notch roof on a lateral knee X-ray in 30 degrees of flexion. The inferior patellar pole should normally lie on or slightly above this line. It is used to assess patella alta (high-riding patella) and patella baja (low-riding patella) and as a reference for patellar tendon graft tunnel placement.

Blumensaat's line is used in ACL reconstruction to position the femoral tunnel at the 10 o'clock (right knee) or 2 o'clock (left knee) position relative to the notch roof. The native ACL footprint lies just below and parallel to Blumensaat's line. Patella alta (Insall-Salvati ratio > 1.2) is associated with patellar instability; patella baja (ratio < 0.8) complicates total knee arthroplasty. The line is best visualised on a true lateral X-ray with 30 degrees of knee flexion.
