Interspinous Ligament Tendinous Band

fasciculus tendineus ligamenti interspinalis

The interspinous ligaments are thin sheets of fibrous tissue joining the lower border of one spinous process to the upper border of the next. In the lumbar spine their fibres run obliquely, and the ligament is described in ventral, middle and dorsal parts that blend with the ligamentum flavum in front and the supraspinous ligament behind.

Clinical Relevance

Clinical Notes

The interspinous ligament tendinous band is the structure implanted around by interspinous spacer devices (X-Stop, Coflex) for lumbar spinal stenosis treatment. Its integrity is assessed by the prone lumbar extension provocation test. Kissing spine syndrome (Baastrup disease) involves degeneration and bursitis at the interspinous tendon complex between adjacent lumbar spinous processes.

Pathology

Common Injuries & Conditions

Baastrup Disease (Kissing Spine)

Degenerative change in the interspinous ligament tendinous band from contact between adjacent spinous processes in lumbar lordosis producing midline lower back pain worsened by extension, managed with extension restriction and interspinous injection.
