It attaches to the posterior sacrum, iliac crest, and lumbar spinous processes, functioning as a common origin for the entire erector spinae group. It blends with the posterior layer of the thoracolumbar fascia to form a powerful force-transfer mechanism across the lumbopelvic region.
The aponeurosis is a common site of strain in heavy lifting injuries. Tears appear as high-signal areas on MRI. Chronic low back pain associated with erector spinae aponeurosis pathology responds to progressive loading programs targeting spinal extensors. Avulsion from the iliac crest may occur in high-velocity trauma.
Partial or complete disruption of the aponeurotic sheet from the iliac crest or sacrum, producing acute low back pain and difficulty in trunk extension, most common in weightlifters and manual laborers.
