The erector spinae aponeurosis is the broad, flat tendinous sheet from which the iliocostalis, longissimus, and spinalis columns arise in the lumbar region. It attaches to the posterior sacrum, iliac crest, and lumbar spinous processes, functioning as a common origin for the entire erector spinae group. It blends with the posterior layer of the thoracolumbar fascia to form a powerful force-transfer mechanism across the lumbopelvic region.

Clinical Notes

The aponeurosis is a common site of strain in heavy lifting injuries. Tears appear as high-signal areas on MRI. Chronic low back pain associated with erector spinae aponeurosis pathology responds to progressive loading programs targeting spinal extensors. Avulsion from the iliac crest may occur in high-velocity trauma.
