The deep fibular nerve supplies all anterior compartment muscles and has a tiny sensory territory — only the first dorsal web space. Anterior tarsal tunnel syndrome from compression of the deep fibular nerve under the inferior extensor retinaculum produces first web space numbness and EDB weakness, distinguishable from L4-L5 root compression by the absence of tibialis anterior and EHL weakness (which are proximal to the tarsal tunnel compression level).

Clinical Notes

Anterior tarsal tunnel syndrome is the least common tarsal tunnel variant — tight footwear pressing on the dorsal ankle compresses the deep fibular nerve under the inferior extensor retinaculum. Diagnosis by Tinel's sign at the ankle dorsum and EMG showing EDB delay. Footwear modification, injection, and retinaculum release resolve most cases.
