The sternothyroid is the deeper strap muscle, attaching directly to the thyroid cartilage. It is the key muscle for laryngeal descent after swallowing.

The sternothyroid is the landmark for the middle thyroid vein in thyroidectomy. Its oblique line attachment to the thyroid cartilage is the reference for the cricothyroid space. In open laryngeal surgery, the sternothyroid is reflected to expose the laryngeal framework.
