Second Plantar Interosseous Detail

musculus interosseus plantaris secundus detail

The second plantar interosseous adducts the fourth toe toward the axis of the foot (third toe axis). The three plantar interossei adduct toes 3, 4, and 5 toward the second toe axis (DAB vs PAD mnemonic).

Nerve: Lateral plantar nerve (S2, S3) Blood Supply: Plantar arch Region: Foot & Ankle
Anatomical Data

Origin, Insertion & Supply

OriginMedial aspect of the fourth metatarsal shaft
InsertionMedial base of the fourth toe proximal phalanx
Nerve SupplyLateral plantar nerve (S2, S3)
Blood SupplyPlantar arch
Biomechanics

Function & Actions

ActionsAdducts the fourth toe toward the third; Flexes the fourth MTP joint
Clinical Relevance

Clinical Notes

Plantar interossei atrophy in intrinsic minus foot from peripheral neuropathy contributes to MTP extension and IP flexion (clawtoe). Individual plantar interosseous testing is by resisted adduction of each individual toe.

Palpation

Not individually palpable. Tested by resisted adduction of the fourth toe.

Pathology

Common Injuries & Conditions

Plantar Interosseous Atrophy in Neuropathic Clawtoe

Plantar interosseous atrophy from diabetic peripheral neuropathy contributing to metatarsal head prominent gait and lesser clawtoe deformity.
