The third tarsometatarsal ligament connects the third metatarsal base to the lateral cuneiform, providing stability to the central TMT complex. The third TMT joint has intermediate mobility between the rigid central joints and the more mobile lateral joints.

Third TMT ligament injury commonly accompanies second TMT disruption in Lisfranc injuries. Homolateral Lisfranc dislocations involve medial column injuries (first and second TMT) while divergent dislocations involve all five TMT joints including the third.
