The volar peripheral attachment of the TFCC connects the palmar margin of the triangular fibrocartilage to the volar rim of the sigmoid notch and to the styloid base region of the ulna at its foveal attachment.

Function & Actions

Provides the volar anchor of the TFCC, resists volar ulnar head displacement (dorsal subluxation of the radius during pronation), and is the critical attachment for foveal TFCC avulsion repair.

Clinical Relevance

Clinical Notes

Foveal TFCC detachment (loss of the volar and deep attachments at the ulnar fovea) is a specific injury producing the most significant DRUJ instability. It is distinguished from ulnar styloid base fractures by the deep location of the avulsion. Arthroscopic or open foveal TFCC reattachment restores DRUJ stability.

Ulnar Styloid Base Fracture with TFCC Disruption

Styloid base fracture disrupting the volar TFCC peripheral attachment — requires ORIF when DRUJ instability persists after fracture reduction.
