The scaphotrapeziotrapezoid (STT) ligament complex connects the distal scaphoid to the trapezium and trapezoid, stabilising the scaphoid-trapezium-trapezoid joint and controlling scaphoid rotation at its distal pole.

Function & Actions

Provides the distal constraint of the scaphoid, limiting scaphoid flexion, and stabilises the trapezioscaphoid articulation — the basis of STT arthrodesis for scaphoid instability. Acts as the distal anchor of the scapholunate ligament chain.

Clinical Relevance

Clinical Notes

STT arthritis is the second most common site of wrist arthritis after the thumb CMC joint. STT fusion is used in DISI instability and scaphoid non-union to control scaphoid rotation. FCR tendinopathy at the wrist may be secondary to underlying STT arthritis.

Degenerative joint disease at the scaphotrapeziotrapezoidal articulation producing radial wrist pain and restricted wrist movement, managed by joint injection or STT fusion.

STT Fusion for DISI Instability

Scaphotrapeziotrapezoid arthrodesis controlling scaphoid rotation in scapholunate dissociation with DISI deformity, stabilising the proximal carpal row.
