The short posterior sacroiliac ligament connects the first and second transverse tubercles of the sacrum to the posterior superior iliac spine, forming the deepest layer of the posterior SIJ ligamentous complex.

Function & Actions

Provides primary resistance to sacral nutation (forward rotation of the sacral base), limits anterior sacral translation, and is the deepest stabiliser of the posterior SIJ.

Clinical Relevance

Clinical Notes

The short posterior SIJ ligament is the structure that provides the posterior "closing" force of the sacroiliac joint during normal gait. Its dysfunction contributes to SIJ pain and instability. Prolotherapy targeting this ligament has been studied for chronic SIJ pain.
