The ovarian ligament (proper ovarian ligament) is a fibromuscular cord connecting the medial ovarian pole to the uterine cornua, running within the broad ligament. It is the remnant of the gubernaculum ovarii.
Anchors the medial ovary to the uterus, limits ovarian mobility, and provides a direct connection between the ovary and uterus. It is divided when the ovary is conserved during hysterectomy.
The ovarian ligament is ligated and divided during salpingo-oophorectomy, entering the broad ligament posterior to the round ligament. Its division during subtotal hysterectomy with ovarian conservation releases the ovary from uterine attachment.
The ovarian ligament can be a point of twisting in ovarian torsion, particularly when an ovarian mass alters the ovarian centre of gravity, managed by emergency laparoscopic detorsion and oophoropexy if the ovary is viable.
