The plantar plate of the hallux MTP joint is a fibrocartilaginous structure on the plantar MTP surface, providing the plantar floor of the great toe MTP joint and supporting the sesamoid bones.

Function & Actions

Provides plantar MTP stabilisation, supports the sesamoids within FHB tendons, resists MTP hyperextension, and distributes plantar load across the metatarsal head during push-off.

Clinical Relevance

Clinical Notes

Turf toe (hyperextension injury) disrupts the plantar plate. Grade III requires surgical repair for return to sport. Sesamoid fractures may accompany plantar plate tears.

Hyperextension injury tearing the plantar plate and FHB insertion, managed conservatively (grades I-II) or surgically (grade III).

Chronic Hallux MTP Instability from Plantar Plate Insufficiency

Progressive cock-up deformity and metatarsalgia from plantar plate incompetence, treated by repair and possible Weil osteotomy.
