The anterior oblique fibres of the medial elbow capsule run from the medial epicondyle to the coronoid process anteriorly, forming a distinct band that reinforces the anterior medial capsule and supplements the anterior bundle of the ulnar collateral ligament.

Supplementary anterior medial elbow stability; limits valgus stress in elbow positions between 30-120 degrees flexion.

Clinical Notes

The anterior oblique capsular band is disrupted in valgus elbow injuries and may contribute to posteromedial elbow impingement when scarred. It is the structure addressed in anterior capsular release for elbow flexion contracture. In UCL reconstruction surgery, the anterior capsule is carefully preserved to maintain anterior stability.

Anterior Medial Capsular Tear

Disruption of the anterior oblique capsular fibres in acute valgus elbow injury producing medial elbow pain and mild flexion instability, managed conservatively or with UCL reconstruction if combined with anterior bundle rupture.
