Sciatic Nerve Periosteal Bursa

A small periosteal bursa at the ischial tuberosity near the sciatic nerve, in the region where the nerve exits the greater sciatic foramen and passes adjacent to the ischial bone. This bursa may be compressed in ischiofemoral impingement and deep gluteal syndrome.

Clinical Notes

The sciatic nerve at the ischial tuberosity level is the site of ischiofemoral impingement, where the nerve is compressed between the lesser trochanter and the ischium in certain hip positions. Bursal irritation in this region contributes to the deep buttock pain of piriformis syndrome and ischiofemoral impingement. MRI demonstrates periosteal oedema and bursal fluid in the ischiofemoral space.

Bursal and nerve compression in the ischiofemoral space producing deep buttock pain and sciatica worsened by hip extension and adduction, diagnosed by MRI and managed by activity modification and ischiofemoral space injection.
