A bursa between the nuchal ligament and the external occipital protuberance at the skull attachment, reducing friction at the apex of the nuchal ligament as it attaches to the inion. It may become inflamed in chronic neck hyperextension postures.

Clinical Notes

Nuchal ligament suboccipital bursitis produces midline occipital pain at the inion, reproduced by palpation of the external occipital protuberance and aggravated by neck extension. It is part of the suboccipital pain complex. Ultrasound-guided injection at the inion targets this bursa alongside the nuchal ligament attachment.
