The deep infrapatellar bursa lies between the posterior surface of the patellar tendon and the anterior tibia (fat pad level), while the superficial infrapatellar bursa lies between the skin and the distal patellar tendon and tibial tuberosity. The deep bursa reduces friction between the tendon and the Hoffa fat pad during knee extension movements.
Deep infrapatellar bursitis produces pain and fullness beneath the patellar tendon, reproduced by resisted knee extension and palpation deep to the distal patellar tendon. It may be confused with Hoffa fat pad impingement, which is lateral to the patellar tendon. Superficial infrapatellar bursitis (clergyman's knee) from kneeling on hard surfaces produces visible swelling over the tibial tuberosity. Ultrasound-guided injection targets the deep bursa specifically. Chronic cases may require endoscopic bursectomy.
