Gluteus Medius Deep Bursa

The deep gluteus medius bursa lies between the deep surface of the gluteus medius tendon and the gluteus minimus tendon, in the interval between the superior facet of the greater trochanter and the underlying minimus. It is one of several bursae in the peritrochanteric space, alongside the subgluteus maximus bursa (subtrochanteric) and the subgluteus medius bursa (between medius and trochanteric facet). MRI distinguishes the specific bursa involved in greater trochanteric pain syndrome.

Greater trochanteric pain syndrome (GTPS, formerly 'trochanteric bursitis') encompasses tendinopathy of the gluteus medius and minimus tendons along with bursitis. The deep gluteus medius bursa between the medius and minimus is specifically inflamed when the medius tendinopathy involves its deep surface. Ultrasound-guided corticosteroid injection should target the specific inflamed bursa (subgluteus maximus, subgluteus medius, or deep gluteus medius) based on imaging findings. Platelet-rich plasma injection and shockwave therapy target gluteal tendinopathy rather than the bursa per se.
