A small bursa at the anterior surface of the tibiofibular syndesmosis, between the anterior inferior tibiofibular ligament and the anterior ankle capsule. It may become inflamed in high ankle sprains and chronic syndesmotic instability.
Anterior tibiofibular bursitis contributes to chronic anterior ankle pain after high ankle sprain. It is part of the anterolateral ankle impingement complex, along with synovial hypertrophy in the anterolateral gutter. Ultrasound identifies fluid at the anterior syndesmosis level. Syndesmotic injection at this level provides both diagnostic confirmation and therapeutic relief in anterior ankle pain after high ankle sprain.
